Technical Assistance:
Discussed the following with the administration:
- timeframe for completing background checks after the initial checks are completed
- reviewed the updated sworn disclosure statement
- possibility of lowering the age range down to 16 months.
- food allergy or intolerance and actions to take in case of emergency

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 5 staff files, 6 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 11:15-2:00pm. There were 60 children under the supervision of 10 staff. During the inspection, children were observed praying for and eating lunch, listening to a teacher read the Nutcracker story, reading books, singing Christmas songs in Spanish, listening to audio books and resting during naptime. The children bring in their lunches from home each day they are in care. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 5 staff certified in MAT (medication administration training). Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-185-130-B
Description: Based on file review, 1 out of 6 children's records do not have additional immunizations every six months up to the age of 2 years.
Evidence
1. Child #5 is 3 years old. The most recent immunization record was updated when the child was 18 months old.

Plan of Correction: The Director called the parent during the inspection, asking to bring this into the center.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 4 out of 6 children's records do not have all required information.
Evidence
1. Children #1, #2 and #6 do not have the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
2. Child #4 has a peanut allergy listed, but there is no action to take in case of an emergency situation.

Plan of Correction: The parents will be asked to include the missing emergency contact information. For the food allergy, this parent will be asked to provide documentation of what action to take.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 5 staff files did not include all required information.
Evidence
1. Staff #3 did not have written information to demonstrate that the individual possesses the education required by the job position.

Plan of Correction: The Director has already asked Staff #3 to bring this into the center.

Standard #: 22VAC40-185-340-A
Description: Based on file review and interview, the center was unable to ensure the care and protection of a child in care.
Evidence
1. Based on the medication authorization, child #7 requires albuterol to be at the center as a medication to be administered on an "as needed" basis. The child is in care today, but this medication is not available at the center to administer in case of emergency.

Plan of Correction: The Director will ask the parent to bring in the medication while the child is in care.

Standard #: 22VAC40-185-510-C
Description: Based on file review, the center failed to follow all procedures for administering medications.
Evidence
1. Children #1, #7 and #8 have medication authorization forms which do not include the duration of authorization for the medications. Each of these forms includes "until further notice" listed under the effective date for the medication authorization.

Plan of Correction: These forms will be updated to include this information.
