Comments:
An unannounced monitoring inspection was conducted from 9:45 am - 12:45 pm with the center director. There were 44 children, ranging from six months to five years of age, supervised by ten staff. Children were observed eating morning snack, playing in centers, listening to a story, and completing seat work. Snack served today: gogurt, graham crackers and water. Five children's records were reviewed and in compliance. Four staff records were reviewed and in compliance. One medication was reviewed and in compliance. The emergency drill log was observed and in compliance. The last health inspection was conducted: 10/3/19 and the last fire inspection was conducted: 10/8/19. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in a safe condition. Evidence: metal stripping is coming off of the stage area in the school age classroom leaving a sharp, pointed edge.

Plan of Correction: Will call maintenance and request that they repair it this evening.

Standard #: 22VAC40-185-340-A
Description: Based on observation, the center did not ensure care and protection of all infants. Evidence: four cribs being used by infants had sheets that were not tight fitting and allowed material to gather on the surface of the mattress presenting a suffocation or strangulation hazard. One infant was observed sleeping with a bib and pacifier attached to a ribbon presenting a strangulation hazard.

Plan of Correction: Parents will be asked to provide appropriate sized sheets. Infants will not be put to sleep with bibs on or pacifiers attached to anything that could be considered a strangulation hazard.

Standard #: 22VAC40-185-500-A
Description: Based on observation, it was determined that not all children washed their hands with soap and water or disposable wipes after eating. Evidence: children in the 3's classroom did not wash their hands after eating morning snack; they went straight to the carpet for story time.

Plan of Correction: Staff will be retrained on ensuring that children's hands are washed before and after eating meals and snacks.

Standard #: 22VAC40-185-520-C
Description: Based on observation, not all diaper ointment was inaccessible to children. Evidence: there was a container of "Buttpaste" and "Desitin" stored on a low, open shelf that was accessible to the infants and toddlers in the room.

Plan of Correction: Moved the diaper ointments to a higher shelf during the inspection.

Standard #: 22VAC40-185-570-A
Description: Based on observation, not all children using infant seats/a feeding table had protective belts fastened. Evidence: a child was observed sitting in a feeding table and the strap was not fastened.

Plan of Correction: Child's strap was fastened during the inspection.
