Comments:
A non-mandated Self Reported Monitoring inspection was conducted today regarding an incident the center self reported to Licensing. Interviews were conducted with Staff regarding the incident.There were 42 children present with 10 staff members, Violations of the Standards are recorded on the Violation notice. Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interview with staff, children under 10 yrs were not always within actual sight and sound supervision of staff.
Evidence:
Staff self reported that a child (14 months) in Infant C was found alone in the hallway by the kitchen after a parent left the classroom door open. Further interviews revealed that Staff were unaware the child had left the room.

Plan of Correction: Parents were reminded to close the doors after leaving the room, as well other Center policies to ensure the safety of children. A full staff training will be held on 1/30/2020 to review CSRs, ratios and health safety procedures.
