An unannounced renewal inspection was conducted on 01/14/2020. There were 8 children, ages 4 years-10 years, and 2 staff members present during the inspection. The inspector reviewed 5 children's records, and the on-site staff records for 2 staff. The staff stated that there is no medication being administered. The children were observed in the following activities: arriving to the program, during homework time, participating in large motor activities, and during afternoon snack. The inspector reviewed the emergency preparedness plans, the parent handbook, the playground safety plan and the emergency supplies during the inspection. The inspector discussed children's allergies, required staff training and large motor play with the staff. Standard #: 22VAC40-185-60-A Description: REPEAT VIOLATION
Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
1. The record for Child #4 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the address for the first designated person is incomplete) and the first date of attendance.
2. The record for Child #5 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the address for the first designated person is incomplete) and the first date of attendance.
Plan of Correction: The program director will get the information from the parent today.
Standard #: 22VAC40-185-560-K Description: REPEAT VIOLATION
Based on observations and discussion with the staff, the facility failed to ensure that tables are sanitized before and after each use for feeding.
Evidence: The tables were not sanitized before or after the afternoon snack. There was no sanitizing product available for use during the inspection.
Plan of Correction: Will ask supervisor for sanitizer..
