63.2(17) License & Registration Procedures

Technical Assistance:
The sworn disclosure statement, search of the central registry (CPS check), and fingerprint check have to be repeated every five years.

TB tests or screenings must be conducted every two years.

Each staff must have 16 hours of annual training each year.

Comments:
An unannounced monitoring inspection was conducted with the center director and staff on January 14, 2020 to determine compliance with the licensing standards. There were five toddlers and 14 two year old children present with four staff actively supervising. The children were eating lunch and preparing for rest. Lunch served today: chicken, broccoli, cheese casserole, applesauce, and milk. Five staff files and five children's files were reviewed - please see Supplemental Summary for details. Five medications for three children were reviewed; two staff are certified in Medication Administration Training. The last health inspection was conducted 10/28/2019. The last fire inspection was conducted 6/26/2019. The last shelter in place drill was conducted 6/6/2019. Standard #: 22VAC40-185-160-C
Description: Based on review of five staff files, the facility failed to have a repeat tuberculosis test or screening for each staff every two years as required. Evidence: Staff 5 did not have a current tuberculosis test or screening on file (expired 6/22/2019).

Plan of Correction: All staff will get a current tuberculosis test or screening prior to expiration.

Standard #: 22VAC40-185-510-A
Description: Based on review of five medications for three children, the facility failed to have written authorization from the parent for the facility to administer each medication. Evidence: there was no written authorization from the parent allowing the facility to administer one medication for Child B.

Plan of Correction: The center will ask the parent to complete a medication authorization form.

Standard #: 22VAC40-185-510-E
Description: Based on review of five medications for three children, the facility failed to label each medication with the child's name, the dosage amount, and the time or times to be given. Evidence: one medication for Child A was not labeled with the child's name, dosage amount, or time to be given.

Plan of Correction: The medication will be labeled with the child's name, dosage, and time to be given.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of four staff files, the facility failed to have repeat background checks every five years for each staff as required. Evidence: Staff 2 (date of hire 2/27/2008) did not have a repeat sworn disclosure statement (expired 11/24/2019) or a repeat search of the central registry (expired 12/22/2019).

Plan of Correction: Staff 2 will complete a sworn disclosure statement and a new search of the central registry request will be sent.
