Comments:
An unannounced monitoring inspection was conducted with center director and staff on January 15, 2020 to determine compliance with the licensing standards. There were 4 preschool children present with one staff and 7 school age children present with two staff. The preschool children were playing outside and dismissing for the day. The school age children were arriving from school, having afternoon snack, and playing outdoors. Snack served today: grape juice and goldfish. Four staff files and five children's files were reviewed - please see Supplemental Summary for details. The last health inspection was conducted 5/1/2019. Standard #: 22VAC40-185-550-E
Description: Based on the emergency drill log and staff interviews, the facility failed to maintain a record of evacuation drills and shelter in place drills for multiple shifts, divided evenly among the various shifts. Evidence: the evacuation drills and shelter in place drills were all conducted and documented during the school/preschool day, but none were noted practiced during the before school and after school program. Two after school staff stated that they have not practiced an evacuation drill during after school care.

Plan of Correction: Evacuation drills and shelter in place drills will be conducted during all shifts of operation, including before and after school.

Standard #: 22VAC40-191-60-B
Description: Based on review of four staff files, the facility failed to have documentation of a signed sworn disclosure statement prior to employment for each staff. Evidence: Staff 3 (date of hire 6/17/2019) did not have documentation of a sworn disclosure statement on file.

Plan of Correction: Staff 3 will complete the sworn disclosure statement.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of four staff files, the facility failed to have documentation of a search of the central registry within 30 days of employment. Evidence: Staff 3 (date of hire 6/17/2019) and Staff 4 (date of hire 6/12/2019) did not have documentation of a search of the central registry on file.

Plan of Correction: Licensing inspector will send the CPS request form and staff will send it in immediately.
