An announced non-mandated record review inspection for the Danville YMCA- Stony Mill was completed on 01/22/2020. The inspector reviewed 2 staff records and 5 Board Officer/Agent records during the inspection today. The inspector arrived at 9:00 a.m. and departed at 11:45 a.m. Thank you for your time today. Standard #: 22VAC40-185-70-A Description: Based on record review and discussion with staff, the facility failed to ensure that staff records shall contain all of the elements required by the standards.
1. The record for Staff #1 did not contain written information to demonstrate that the individual possesses the education, staff development, certification and experience required by the job position. The record for Staff #1 had a documented job title of program lead and the record did not contain the written information to demonstrate that Staff #1 was qualified for that position.
2. The record for Staff #2 did not contain written information to demonstrate that the individual possesses the education, staff development, certification and experience required by the job position. The record for Staff #2 identifies Staff #2 as the Child Care Coordinator which is the organizations term for Program Director.
Plan of Correction: The information will be obtained and placed into the record.
Standard #: 22VAC40-191-60-B Description: Based on record review and discussion with the staff, the facility failed to ensure that no person shall be employed until the agency has the persons completed sworn statement or affirmation.
Evidence: The record for Staff #1 had a documented hire date of 09/03/2019; the sworn statement was date 10/16/2019.
Plan of Correction: Sworn statements will be completed before the first day of work.
