Description: Based on record review, the vendor failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's sensitivities, and dietary restrictions.

Evidence:
1. Child 1 has a food sensitivity to milk and a dietary restriction to milk as a result.
a. This food sensitivity and dietary restriction is not listed on the facility food allergy list.

Plan of Correction: This information was added to the food allergy list during the inspection.

Standard #: 22VAC40-665-520-B-11
Description: Based on record review and interview, the vendor did not have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
1. The physical and enrollment form for child 2 and child 3 indicates a diagnosed food allergy to shellfish.
a. There is no written allergy care plan on file for child 2 and child 3.
2. The program director confirmed that a written allergy care plan, written by the physician of child 2 and child 3, was not on file.

Plan of Correction: The parent of child 2 and child 3 will be asked to provide a written allergy care plan, from the child's physician, within the next two weeks.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the vendor failed to ensure that the documentation of the required tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The TB screening for staff 1 was completed on 05/13/2019.
a. Staff 1 has a hire date of 11/18/2019 therefore, the screening was conducted outside of the 30 day time frame.

Plan of Correction: Going forward, all new staff will obtain a TB screening within 30 days prior to employment.
