Comments:
The licensing inspector conducted an unannounced monitoring inspection on 0/128/2019 from 10:18am to 2:30pm. The children were observed learning through play in centers, creating crafts, playing instruments, dancing, having tummy time, playing outside and eating lunch. A formal observation was completed with the pre-k and two year old classrooms. Lunch consisted of macaroni and cheese, bread, pears, peas and carrots, salad with dressing and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the center's mini buses.

Five children?s records and five employee records were reviewed during this inspection. The center administers medication and medication was reviewed.

Five injury reports were reviewed.

The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:
Infants - 10:3
Toddler ? 5:1
Twos - 10:2
Twos and Threes - 16:2
Preschool and Pre-K - 13:2


The violations from the previous inspections were checked for corrections. There were two repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not ensure two of five children's records had documentation of immunizations before the child can attend the center.
Evidence:
1. The record of child #1 (start date: 1/28/20) did not have documentation of immunizations.
2. The record of child #4 (start date: 9/23/19) did not have documentation of immunizations.

Plan of Correction: We have immediately sent out letters informing parents they can not return in 5 business days without missing documentation.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not ensure one of five children's records had documentation of a physical examination within 30 days of attendance.
Evidence:
1. The record of child #5 (start date: 9/3/19) did not have documentation of a physical examination.

Plan of Correction: We have immediately sent out letters informing parents they can not return in 5 business days without missing documentation.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure one of five children's records had all of the required documentation.
Evidence:
The record of child #2 (start date: 12/2/19) did not have documentation that the center viewed the child's proof of age and identity.

Plan of Correction: We have immediately sent out letters informing parents they can not return in 5 business days without missing documentation.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The school age, preschool, pre k and toddler classrooms had areas with chipped and peeling paint.
2. The toddler classroom had two holes in the wall which exposed crumbling drywall.
3. The preschool playground chain link fence had a bungee cord and jump rope connected to the fence presenting and entanglement hazard.-

Plan of Correction: The work order was placed in October and will be completed soon.
