Comments:
A Subsidy Health and Safety inspection was conducted on 0/128/2020 from 10:18am to 2:30pm to review supplemental health and safety requirements. The children were observed learning through play in centers, creating crafts, playing instruments, dancing, having tummy time, playing outside and eating lunch. A formal observation was completed with the pre-k and two year old classrooms. Lunch consisted of macaroni and cheese, bread, pears, peas and carrots, salad with dressing and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the center's mini buses.

Five children?s records and five employee records were reviewed during this inspection. The center administers medication and medication was reviewed.

Five injury reports were reviewed.

The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:
Infants - 10:3

Twos and Threes - 16:2
Preschool and Pre-K - 13:2


A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation of the supplemental subsidy health and safety inspection was found.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the center did not ensure one of five staff records contained documentation of a tuberculosis screening at the time of employment and before contact with children. The documentation should have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
The record if staff #2 (start date: 8/19/19) had documentation of TB results completed in May of 2019.

Plan of Correction: Employee was sent to the doctor, paid for by the company. In the future, employer will ensure request for testing is granted.
