Technical Assistance:
Discussed requirements for children records contain parents employers address, physical plant concerns about bolt covers on toilets and the panel/bead board roughness and splintering.

Comments:
Licensing inspector arrived at 1:00 pm and departed at 2:32pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there were 62 children in care with 12 staff . Additional staff arrived during the inspection. Children were observed napping. The sample size consisted of 5 children records and 5 staff records. Areas of non-compliance was reviewed with the director.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the licensee did not ensure children records contain all required information.
Evidence:
Child #1 and child #5 did not have one parent's employer phone number.

Plan of Correction: Go through files and add employer address,. We have added employer address to enrollment form.

Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interview, the licensee did not ensure areas of equipment of the center inside, was maintained in a clean safe and operable condition.
Evidence:
In the boys restroom, one toilet did not have a toilet tank cover.

Plan of Correction: Bathroom will be out of order until part comes in for toilet.
