Comments:
An unannounced monitoring inspection was conducted on Friday, December 6, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 10:40 AM and concluded the inspection at 1:15 PM. The census for today's inspection consisted of 34 children. During the inspection, children were coloring, practicing shapes, singing, having circle time and story time, playing outside, bathroom breaks, washing hands, eating lunch and napping. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Five children's records, five staff records, annual fire and health inspections, fire drill and shelter in place drill logs, daily attendance and all required postings were reviewed.

A member of management was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records, the center did not ensure that each staff member shall submit
documentation of a negative tuberculosis screening (TB) no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Evidence:
1. The records for staff #1 (date of hire 11/07/19) and staff #4 did not have documentation of the TB screening/test.
2. The regional director confirmed that the screening was not in the record for staff #1 and staff #4.

Description: Based on observation and teacher interview, the facility did ensure that areas used by children of preschool age and younger shall have protective covers on all electrical outlets.

Evidence:
The electrical outlets were not covered in classroom #4. Children in this classroom are preschool age.
