A Subsidy Health and Safety inspection was conducted on January 28, 2020 to review supplemental health and safety requirements. Two staff were present with thirty-seven children at the onset of inspection. A third staff arrived during inspection. Four children records were reviewed. Required information to be maintained on site and staff training was reviewed for the three staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11 Description: Based on review, facility did not ensure there is a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physical regarding the food the child is allergic to and steps to be taken in the even of a suspected or confirmed allergic reaction.
Evidence: The record for child #2 has documentation of a food allergy on the physical form and there is not a allergy action plan.Plan of Correction: Will obtain allergy action plan.
