Comments:
The licensing inspector conducted an unannounced monitoring inspection on 01/15/2020 from 10:33am to 4:55pm. The children were observed learning through play in centers, eating lunch and preparing for nap. A formal observation was completed with the infant and pre kindergarten classrooms. The menu was posted. Lunch consisted of pasta with tomato sauce, chick peas, oranges, green beans and milk. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today.

Seven children?s records and nine employee records were reviewed during this inspection. Medication is being administered and was reviewed.

The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:
Infants - 15:4
Toddlers ? 12:3
Twos - 15:3

Pre - Kindergarten- 20:2

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The school age classroom had torn soft furniture exposing foam, presenting a choking hazard.
2. The toddler classroom had torn soft blocks exposing foam, presenting a choking hazard.
3. The twos and toddler classroom had areas with chipped and peeling paint.
4. The infant classroom floor had two areas on the floor with missing laminate. One of the areas had a piece of sharp metal.
5. The pre-kindergarten classroom had a radio plugged into the wall with the cord hanging, which presented an entanglement hazard and the radio could fall on a child if pulled.

Plan of Correction: 1.All chairs have been removed as of 1/16/20.

2. All soft blocks removed 1/16/20.

3. A work order was placed on 1/16/20 to fix chipped paint.

4. Work order was placed on 1/16/20 to fix infant floors.

5. Position of the radio was changed to ensure safety 1/16/20.

Health and Safety coordinator will monitor the condition of furniture monthly.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The hallway maintenance closet was unlocked. The closet contained a chemical within the reach of children.
2. The Twos classroom had an unlocked cabinet containing cleaners, out of the reach of children in care.
3. The School Age classroom had an unlocked cabinet containing cleaners, out of the reach of children.

Plan of Correction: All cabinets have been locked and staff were retrained by center director on hazardous substances

Standard #: 22VAC40-185-510-E
Description: Based on observation and record review, the center did not ensure that medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
The infant classroom had a tube of unused medication located in a basket with the diaper cream. The medication was not labeled with the child's name, name of medication, dosage amount and the time or times to be given.

Plan of Correction: Medication has been removed and process has been developed to check for medication and the correct forms to ensure accuracy

Standard #: 22VAC40-185-520-C
Description: Based on observation and record review, the did not ensure diaper ointment had all of the required documentation.
Evidence:
The infant class had several tubes of diaper cream that did not have the required parent permission documentation.

Plan of Correction: All required documentation has been received for medication in infant room and staff were retrained by center director on policy regarding medication authorizations.

Standard #: 22VAC40-185-570-C
Description: Based on observation, the center did not ensure to obtain the required documentation for each child on formula.
Evidence:
The infant children in care did not have documentation of their brand of formula and feeding schedule.

Plan of Correction: All infant children have the required paperwork to show formula and feeding schedule- there is a new system in place to ensure these documents are collected.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, the center did not ensure two of nine staff records contained documentation of out of state Child Abuse and Neglect search request by the end of the 30th day of employment and a follow up 45 days after request to search.
Evidence:
1. The record of staff #1 (start date: 11/1/2019) did not have documentation that the center conducted an out of state child abuse and neglect search request.
2. The record of staff #2 (start date: 4/12/19) did not have documentation that the center followed up on the staff's out of state search request within 45 days after the initial request.

Plan of Correction: 1. Resubmitted out of state background check on 01/16/19

2. Reprinted out of state background check (originally dated 08/20/2019).
