Comments:
An announced inspection was conducted on February 4, 2020, at the request of the licensee for a modification to the license to increase the capacity. During the inspection additional program space and restrooms were inspected. The gymnasium, childwatch room and program classroom #1 were observed and measured in the "Main Building". A second building, the "Childcare Cottage" was also observed and measured. The wellness room in the "Program Center", a third building was also measured. The area of Special Care Provisions/Emergencies were also reviewed for the additional program space. The request for the modification will be reviewed with the Licensing Administrator.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: One of three buildings, the "Childcare Cottage" has a broken concrete stair leading to the entrance of the building. The broken portion of the stair exposes a hole in the stair that is between 5 and 6 inches wide and there are sharp edges around the hole.

Plan of Correction: The center director will ensure it is repaired. The director will close off the stairs with caution tape until it is repaired.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In one of three buildings, the "Childcare Cottage" in the restroom the following chemicals were observed unlocked.
1) In the unlocked cabinet there was a container of cleaner and air freshener.
2) On the open shelf above the toilet there were 4 containers of cleaner and 1 container of paint.
All containers were labeled "Keep out of reach of children" and at least one of other statements "hazardous", "flammable" and "caution".

Plan of Correction: The chemicals will placed in a locked cabinet.

Standard #: 22VAC40-185-280-G
Description: Based on observation and staff interviews, the licensee did not ensure if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents and shall not resemble food or beverage containers.

Evidence: In one of three buildings, the "Childcare Cottage" under the unlocked cabinet in the restroom there was a spray container with a clear liquid and the container was not labeled with it's contents. Center director indicated it was bleach and water.

Plan of Correction: The center director will get rid of the bottle.

Standard #: 22VAC40-185-550-C
Description: Based on documentation review and staff interviews, the licensee did not ensure the center's emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building.

Evidence: One of three buildings, the "Childcare Cottage" did not have an emergency evacuation and shelter-in-place procedures posted.

Plan of Correction: The center director will post procedures tomorrow.
