Remember: All staff are required to attend 16 hours of training annually.

Comments:
An unannounced monitoring inspection was conducted today from 9:30 am until 1:30 pm. There were 60 children present with 16 staff member. The children were observed working on are projects and listening to storytime. The were also observed during hand-washing and diaper changing. The entire physical plant, attendance logs,evacuation logs, emergency plans, menu's cribs, bottles, children's records, staff records, emergency supplies and medications were inspected. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the center failed to ensure the retain required documentation.

Evidence:
1.Child #2's record does not have the names of person authorized to pick up the child.

Plan of Correction: I will the information from parents today.

Standard #: 22VAC40-185-210-A-1
Description: Based on record review, Program Leaders did not have documentation on site to verify qualifications.

Evidence:
Staff #2 and #4 did not have documentation in their records qualifying them as Program Leaders.

Plan of Correction: I will place documentation in staff records.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
1. The dark blue mats in the Infant Room are frayed on the edges and are unraveling which could cause a chocking hazard.
2. In the Jr. Preschool Room the floor tiles are missing near the sink and the backdoor at the rear of the classroom.

Plan of Correction: 1. The mats have been ordered for the Infant Room.
2. The floor tiles will be replaced this weekend.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by the children.

Evidence:
1.In the adult bathroom, disinfectant spray and and bleach water was located on a shelf, not kept in a locked place using a safe locking method that prevents access by children.
2.In the Toddler Room, a disinfectant spray, a bleach water solution, bleach and bleach wipes were located under a sink and not kept in a locked place using a safe locking method that prevents access by children.
3. In the Jr.Preschool Room, a disinfectant spray, a bleach water solution and a window foaming cleaner were located on a shelf were located on a shelf and not kept in a locked place using a safe locking method.

Plan of Correction: There items were placed in a locked cabinet during the inspection.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center failed to ensure that if diaper ointment or cream is used, that product does not need to be kept locked but shall be inaccessible to children.

Evidence:
1.In the Infant Room, diaper creams are stored under the changing table and are accessible to children.
2.In the Jr. Preschool Rood, diaper creams are stored under the changing table and are accessible to children.

Plan of Correction: Diaper creams were removed from under the changing tables during inspections and placed in a space inaccessible to children.

Description: Based on observation, the findings of the most recent inspection were not posted.

Evidence:
The violation notice from the most recent inspection is not posted.
