Technical Assistance:
Ratios should not be doubled at naptime until the vast majority of children are resting/asleep on their cots.

Comments:
An unannounced monitoring inspection was conducted beginning at 1:10 p.m. and ending at 5:15 p.m. with the principal. There were 90 children age 2 months to 5 years of age under the care and supervision of 10 staff. Infants were observed sleeping, being fed and having playtime on the floor. Toddlers, two year olds, three year olds, four year olds and five year olds were observed during rest time. Classrooms, supplies and materials were observed. The playgrounds were not inspected due to the weather.

Nine children's records and seven staff records were reviewed. Five medications and authorizations were reviewed. Emergency drills were up to date.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of nine children's records, it was determined that the center did not obtain documentation of a physical exam within 30 days of attendance. Evidence: Child #6, enrolled 5/30/19, did not have documentation of a physical in the record.

Plan of Correction: This information was again requested from the parents during the inspection.

Standard #: 22VAC40-185-60-A
Description: Based on review of nine children's records, it was determined that the center did not maintain all required information. Evidence: Child #6, enrolled 5/30/19, did not have an address for a second emergency contact. Child #10, enrolled 5/1/2019, did not have a second emergency contact and parents' employers information.

Plan of Correction: This information was again requested from the parent's during the inspection. Child #10 information was updated during the inspection.

Standard #: 22VAC40-185-320-B
Description: Based on observation, it was determined that not all restroom areas provided to children were equipped with required items. Evidence: In the women's bathroom in the Junior Kindergarten/School-Age Clubhouse classroom, there was no toilet paper or disposable paper towels.

Plan of Correction: This was corrected during the inspection. Teacher's will be reminded to address any supply needs.

Standard #: 22VAC40-185-510-A
Description: Based on review of medication, it was determined that the center failed to follow it's written medication policy as required.
Evidence:
1. Child #1 had parent's authorization for children's motrin signed on 12/10/2019. The center's policy requires parental authorization to be renewed within 7 days or the medication sent home.
2. Child #3 has parent's authorization for children's benadryl signed on 12/9/2019. The manufacturer's instructions state "Do Not Use" for children under the age of 2 years old. The child's date of birth is 5/9/2018. The center's policy states "No medication will be dispensed in a manner that is contrary to the label directions wihtout a physician's written instructions.
3. Child #3 parent's authorization for children's benadryl was signed on 12/9/2019. The center's policy requires parental authorization to be renewed within 7 days or the medication sent home.

Plan of Correction: Medications are being sent home today.
