Technical Assistance:
As of the Inspection date, the center renewal application and fee has not been received. Please submit prior to expiration.

Comments:
An unannounced renewal study was conducted today from 11:45-1:30. All rooms were in ratio with direct care staff. Observed rest time, infant care, transition to the bathroom/hand washing, lunch and diapering. Lunch served today was pizza, banana and salad. Reviewed annual liability insurance coverage, records, injury updates, certifications and training for staff. Emergency medication is on-site with authorization and storage up to date. Several staff have Medication Administration Training (MAT) certifications as well as Daily Health Observation (DHO). Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 6 children's records did not contain all required information.
Evidence:
1. Child #3's record did not contain proof of identity within 7 days of enrollment (1/9/2020)
2. Child #4's record did not contain proof of identity within 7 days of enrollment (1/6/2020)

Plan of Correction: Proof was documented before the back up service children (#3 and #4) were allowed to return.

Standard #: 22VAC40-185-280-B
Description: Based on observation, cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
A container of bleach water was in an unlocked cabinet in the Preschool 2 classroom.

Plan of Correction: Retrained staff to ensure they are fully engaging locks, even when they are at a height of 5 feet.

Standard #: 22VAC40-185-450-A
Description: Based on observation and staff statement, an occupied cot did not have both a top and bottom cover.
Evidence:
A child laying on a cot did not have a top cover. The staff member confirmed that the parent had not bought one into the program.

Plan of Correction: A cot cover was immediately provided. Parents received a final notice that a cover must be brought in weekly.
