Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:15pm. There were 68 children directly supervised by 16 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing in centers with manipulatives, building with blocks, working on letters, listening to music and dancing. The infants were in the midst of nap time. Handwashing and diaper changing were observed. Interactions between children and staff were observed and deemed to be positive. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas inside were not maintain a clean and safe condition.

Evidence:
1) In the Pre K classroom, there were several areas (window sills and block area) along the wall with peeling paint.
2) In the school age classroom, there was peeling paint in the windows of the library area and in the boys bathroom on the left side.
3) In the infant B classroom, there is a hole about the size of a quarter underneath of the first window.

Plan of Correction: Work orders have been previously placed. Director will follow up with work orders to find out the status of them.

Standard #: 22VAC40-185-440-A
Description: Based on observation and record review, the center did not have cots identified for use by a specific child.

Evidence: In Preschool and Pre K classroom, their cots were not identified for use by a specific child. There were no names or numbers assigned to the cots to know which belonged to a specific child.

Plan of Correction: The cot charts are usually labeled;however, the director will relabel the cot and make a cot chart.
