Comments:
An unannounced monitoring inspection was conducted on March 5, 2020 during the after care portion of the program. There were two staff present with 35 school age children. The children were observed doing homework, playing with board games, legos, blocks, cards and talking freely amongst themselves. Five children's records were reviewed on this date. Required staff information was reviewed for two staff members. Five medications were reviewed. Licensing Inspector reviewed supervision, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports.

Violations:
Standard #: 22VAC40-185-260-B
Description: Based on observation and inspection of the facility, the licensee did not ensure to obtain annual approval from the health department.

Evidence: The Staff Member in charge confirmed that the most recent documentation of the health inspection report was more than one year old and was dated 9/27/18.

Plan of Correction: We are working with the school to try to obtain a current copy.

Standard #: 22VAC40-185-510-C
Description: Based on review and inspection of the facility, the center did not follow their outdated medication policy.

Evidence: In the center's medication box, there was a medication that expired in October 2019. The center's policy states the following: All medications must be checked for expiration monthly. The center is to notify parents that medications are soon expiring 14 days prior to expiration. The center is to complete a Medication Return slip and return the medication to the parent. If the medication is not returned to the parent, the center is to complete a medication disposal slip prior to disposal. It further states that outdated medications should not be left at site locations.

Plan of Correction: Staff Member will talk to the parent at pick up today and let her know that the medication has expired. The two options to the parent is to return the medication or have her to fill out paperwork from the physician.

Description: Based on observation and staff interview, the licensee did not ensure to have all required emergency supplies at the center.

Evidence: The Staff Member in charge confirmed that there was not an ice pack in the first aid kit.

Plan of Correction: Staff Member in charge will go to the office and get some ice packs and if not ice packs will be purchased.
