Comments:
An unannounced monitoring inspection was conducted on June 22, 2016. Upon arrival to the center, there were 51 school age children present with five staff members. There were additional staff on site for administrative and support purposes. The licensing inspector reviewed emergency evacuation and procedures, rest room and furnishings, administration, emergency supplies, the physical plant, equipment, and medications. The children were observed having lunch, talking freely and listening to music. The children transitioned throughout the day in a variety of activities to include swimming, kayaking, flag football, sand soccer and art on the beach. Four children's records and ten staff records were reviewed. Seven medications were reviewed. The center has a total enrollment of 115 children.

This inspection was amended on July 12, 2016.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, in one of ten staff records reviewed, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: During the inspection conducted on June 22, 2016, the PPD screening obtained for Staff #1 was dated 2/11/15 which had been completed more than12 months prior to the employee's start date which was 5/31/16.

Plan of Correction: I will have the staff member complete a TB before returning to work.

Standard #: 22VAC40-185-510-C
Description: Based on observation and review of medication, the licensee did not ensure that the center's procedures for administering medication shall have methods to prevent use of outdated medication. Evidence: During the inspection conducted on June 22, 2016, there were two expired medications for Child #2. One medication had an expiration date of September 2015 and the other medication had an expiration date of January 2016.

Plan of Correction: The medication will be sent home with the parents today. We will talk to parents about bringing in updated medication.

Standard #: 22VAC40-185-510-E
Description: Based on review and inspection of the facility, the staff members did not ensure that medication was labeled with all requirements. Evidence: During the inspection conducted on June 22, 2016, medication for Child #1 was not labeled with the child's name.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-550-D
Description: Based on review and inspection of the facility, the center did not implement a monthly practice evacuation drill and a mininum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on June 22, 2016, there was no documentation of an evacuation drill practiced in August 2015 and no documentation of shelter-in-place drills in 2015.

Plan of Correction: The evacuation and shelter in place drills were conducted in 2015, however, the documentation was discarded.
