63.2 Licensure and Registration Procedures
An unannounced monitoring inspection was conducted on October 17, 2017. Upon arrival to the facility, there were 28 children present with three staff members. Additional staff were on site for administrative and support purposes. The preschool children were returning from a field trip to the pumpkin patch and were observed finishing their lunches, napping and playing in their classrooms. The children in the Infant and Toddler Classrooms were observed napping; one infant was awake and was preparing to be fed. The center does administer medication and offer transportation. Five children's records and five staff records were reviewed.
Standard #: 63.2-1716-A-3-a Description: Based on observation and inspection of the facility, the center did not ensure that ratio requirements were maintained for children from birth to two years: one staff member for every four children. Evidence: Upon arrival to the Infant classroom, there was one staff member with five infants. The second staff required for the room was on a scheduled break as verified by the Director. Plan of Correction: We will have one of the two transitioning children to rest in the Toddler room. The second staff person required in the room came back during the inspection.
Standard #: 63.2-1716-A-4 Description: Based on review, in one of five staff records reviewed, the facility did not ensure that in each person in a supervisory position has been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision. Evidence: During the inspection conducted on October 17, 2017, there was no documentation that Staff #4 had been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision. Plan of Correction: The form will be in the staff member's file by tomorrow.
Standard #: 63.2-1716-A-6 Description: Based on review, the facility did not ensure that the following aspects of the child day center's operations are described in a written statement provided to the parents or guardians of the children in the center and made available to the general public: physical facilities, enrollment capacity, food services, health requirements for the staff and public liability insurance. Evidence: During the inspection conducted on October 17, 2017, the following aspects of the center were not described in a written statement provided to the parents or guardians and made available to the general public: physical facilities and public liability insurance. Plan of Correction: The handbook will be updated.
Standard #: 63.2-1724 Description: Based on record review, in five of five staff records reviewed, the facility did not ensure that any child day center that is exempt from licensure pursuant to ? 63.2-1716 shall require all applicants for employment, employees, applicants to serve as volunteers, and volunteers and any other person who is expected to be alone with one or more children enrolled in the child day center to obtain a background check in accordance with ? 63.2-1720.1. Evidence: The following background check information was not documented in each staff member's record: There was no documentation of a completed sworn statement or affirmation, results of a Virginia State Police name search for criminal history and results of a Central Registry Child Protective Services for the following staff Staff #3 (10/31/16) Staff #4 (7/28/17) Staff #5 (9/5/17) There was no documentation of a results of a Virginia State Police name search for criminal history and results of a Central Registry Child Protective Services for the following staff: Staff #1 (Hire Date 6/22/09) Staff #2 (Hire Date 12/1/14) Plan of Correction: Director will locate the background checks for veteran staff. The background checks for the new employees have been submitted and were returned and will be resubmitted upon verification of accurate address.
