Comments:
The Licensing Inspector for Fresh Start Ministries (God?s Creative Learning Center) conducted an unannounced Monitor Inspection on Wednesday, March 28, 2018. Upon arrival at 11:30 AM there were a total of twelve (12) children under the direct care & supervision of two (2) caregivers. Children & staff records were reviewed during today?s inspection. In addition, staff records that were found out of compliance during the previous inspection were reviewed and have all been corrected. Licensing Inspector conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director/Administrator on March 28, 2018. Standard #: VENDSUB-000-018
Description: Based on inspection, the Vendor failed to maintain resilient surfacing (mulch) underneath and surrounding playground equipment. EVIDENCE: Playground equipment with climbing and/or moving parts did not have a least 6 inches of resilient surfacing underneath and surrounding (trajectory areas) the equipment.

Plan of Correction: Playground ground equipment will not be used until additional mulch is added and/or removal of equipment.
