Technical Assistance:
Inspector and provider discussed: groupings and clarifying contact numbers for parents.

An unannounced monitoring inspection was conducted today from 10:00am-11:45am. There were 70 children directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, 1 medication, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, doing an art project, climbing the rockwall, and playing in the gym. Transitions observed were orderly and structured. Counselors counted children before and after transitions to ensure accountability and the presence of each child. The center was clean and organized. Description: Based on record review, the program did not identify the specific sunscreen to be used for each child. Evidence: Five children's files have written parent authorization to apply sunscreen but do not specify the sunscreen for each child.

Plan of Correction: Each parent brings their own sunscreen with the child's name on it. We only use it 3 times over the summer.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, the provider did not complete a central registry check for an employee. Evidence: Staff A (hire date: 6/2/17) has a central registry background check which is not completed. It was sent back to the provider requiring additional information but was not filled out or sent back and therefore is not complete.

Plan of Correction: Sent background check on 7/13.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation, the center did not have the most recent violation notice posted. Evidence: The violation notice posted was from July 7, 2016 and the most recent visit at the facility was conducted on August 5, 2016.

Plan of Correction: Took down old violations and posted the newest one.
