An unannounced monitoring SHSI inspection was conducted today, 8/9/18. The school age children were playing in the gym and the dining/classroom area. The pre-school children went to a local park to play. Lunch and hand washing was also observed. A review of staff and children's records was also conducted. There are a total of 93 children in care with 16 staff present ranging in age of 3 years to 12 years. We discussed the upcoming subsidy training scheduled for the 21st. Should you have any questions please do not hesitate to contact licensing inspector. Inspection began: 9:15 am ended 11:45 pm.
Standard #: VENDSUB-000-016-A Description: Based on observation the vendor failed to ensure that all hazardous substances are kept in a locked place. Evidence: 1. The purple storage cabinet in the preschool room had a door missing. Inside the cabinet on a shelf was bottles of Adhesive spray, rubbing Alcohol and Lysol spray. These items were not locked and accessible to children. Plan of Correction: All hazardous substances were locked during inspection. The assistant director will insure that all staff keep all hazardous substances locked.
Standard #: VENDSUB-000-018 Description: Based on observation the vendor failed to maintain the resilient surfacing under the swings. Evidence: 1. The surfacing under the swings measures approximately 2-3 inches. Plan of Correction: The director will insure that the surfacing is raked to the appropriate depth prior to the children playing on this equipment.
