An unannounced monitoring inspection was conducted on August 16, 2017. Upon arrival to the center, there were 76 school age children present with seven staff members. There were additional staff on site for administrative and support purposes. There is a total enrollment of 104 children. The licensing inspector reviewed emergency evacuation and procedures, rest room and furnishings, administration, emergency supplies, the physical plant, equipment, and medications. The children were observed talking and playing freely amongst themselves, engaging with their counselors, playing outdoors, washing hands and having lunch. There were multiple activities to include relay racing, biking, basketball and traditional camp games. Five children's records and eight staff records were reviewed. One medication was reviewed along with injury report documentation.
Standard #: 22VAC40-185-70-A Description: Based on record review, in one of eight staff records reviewed, the licensee did not ensure that each staff person's record contained all required information. Evidence: During the inspection conducted on August 16, 2017, a reference check taken over the phone for Staff #8 did not include the signature of the person who conducted the check. Plan of Correction: A new reference will obtained for Staff #8.
Standard #: 22VAC40-191-40-D-1-C Description: Based on record review, in one of eight staff records reviewed, the licensee did not ensure that any employee who is required to have background checks receive them before three years since the dates of the last sworn statement or affirmation, most recent central registry finding and most recent criminal history record check report. Evidence: During the inspection conducted on August 16, 2017, the most recent sworn statement or affirmation for Staff #8 was dated 5/27/14. Plan of Correction: The sworn statement for Staff #8 will be updated.
