63.2 Licensure and Registration Procedures

Comments:
Today an unannounced monitoring visit was conducted with the Director. This visit took place in the morning there were 1 child present with 1staff within the supervision guidelines. The child were observed doing legos/blocks, having snack time, and free play in the gym. Positive interactions were observed during this inspection. Snack consisted of pretzels and lemonade. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-160-A
Description: Based on review of the staff records, one staff record did not include documentation of TB Screening was submitted no later than within 21 days after employment. Evidence: Staff #1's record did not include Tb screening( Hire date 10/5/17)

Plan of Correction: Summer director stated that she completed her TB screening however she just need to bring in the form.

Standard #: 22VAC40-185-70-A
Description: Based on review of two staff records, the center did not maintain complete staff records. Evidence: Staff #1's record did not include documentation of orientation training.

Plan of Correction: Summer director stated that staff will complete the training by Thursday.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, the following areas of the center were found not being maintained in aclean, safe and operable condition. Evidence: Chipped/peeling paint were found along the walls in the gym and along the desk.

Plan of Correction: Staff stated that it will be repainted tonight.

Standard #: 63.2-1720.1-B-2
Description: Based on review of records and interview with the provider, a caregiver record did not include a Criminal Record check within 30 days before employment at the facility ends. Evidence: 1. The record for Caregiver #1, who was hired on 10/17, did not include documentation of the results of a Criminal record check. 2. Staff confirmed a criminal record check was mailed off on June 15, 2018 and fingerprints were conducted yesterday Staff #1 and they are just awaiting the results.

Standard #: 63.2-1720.1-B-3
Description: Based on interview with Staff and review of staff records, the center did not obtain a Out-of-state Central Registry check for staff. Evidence: 1. Staff #1's Sworn Disclosure Statement confirmed that they have lived out of the state. 2. Record for Staff#1 (hired 10/2017) did not have documentation of an out-of-state Central Registry check completed prior to employment. 3. Center staff confirmed that an out-of-state central registry check was mailed on June 15, 2018 for staff #1.

Plan of Correction: Staff stated that they are awaiting the results.
