An unannounced monitoring inspection was conducted today, 11/14/18. A review of staff and children's records was conducted. There are a total of 39 children in care with eleven staff. The children were napping during the inspection. Following nap time the children had snack. Please provide to me within 10 days proof that the violations have been corrected. The inspection began: 1:10 pm and ended 3:20 pm approximately. Should you have any questions please do not hesitate to contact me.

Description: Based on review of staff records and interviews the center did not have the Central Registry checks within 30 days of employment. Evidence: 1. Staff # 4, 15 & 16 have hire dates of 6/20/18, 5/29/18 & 9/9/18 and they do not have on file a Central Registry background check.

Plan of Correction: These forms sent off this week. The director will ensure that these are completed upon hire.
