Technical Assistance:
Technical Assistance was offered regarding the following: background checks, staff records and emergency drill plan.

An unannounced monitoring inspection was conducted from 10:30am to 12:15pm. There were 8 children present with three caregivers. The children were observed completing a beading project, participate in a fire drill and do music and movement activities in the gym area. The center conducted a fire drill during the inspection. 5 children's records and 4 staff records were reviewed. There were no medications on site. Standard #: 22VAC40-185-70-A
Description: Based on record review and staff interview, the center did not maintain complete staff records. Evidence: 1. 2 reference checks were missing for Staff # 2 and Staff # 3 from the previous licensing visit on 7/11/19. The center did not complete 2 reference checks for Staff # 2 and Staff # 3 as of this date. 2. Staff orientation was not completed for Staff # 2 as per the licensing visit n 7/11/19. The orientation was not done and documented as of this date.

Plan of Correction: The references will be done and placed in the staff member's record and orientation will also be completed.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and staff interview, the center did not obtain central registry results for employees within 30 days of employment. Evidence: 1. Staff #1, Staff # 2 and Staff # 3's records were missing the central registry findings. Staff # 1 stated that she mistakenly mailed the state police forms for all three employees instead of the central registry forms. Central registry findings for the three staff members were not obtained.

Plan of Correction: The forms are being mailed today. A follow up will be conducted with the inspector once the results are obtained.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and staff interview, the center did not obtain out of state central registry findings for employees that have resided outside of the state of Virginia in the last five years. Evidence: 1. Staff # 1's record indicated that she has resides outside of Virginia, and has lived in another state within the last 5 years. 2. Out of state central registries for the two states were not obtained when the requirement went into effect, and the findings were not available for review during the 7/11/19 inspection. 3. Staff # 1 stated that she thought the fingerprint findings would cover the out of state findings. Staff # 1 did not submit the outs of state central registry forms.

Plan of Correction: The out of state forms will be completed and submitted as soon as possible. A follow up will be conducted with the licensing inspector once the results have been obtained.
