Comments:
A Subsidy Health and Safety inspection was conducted on Monday, March 4, 2019 to review supplemental health and safety requirements. Licensing Inspector arrived & started inspection at 12:40 pm. There were nineteen (19) children ages 10 months-4 years old present under the direct care of four (4) staff members. The Licensing Inspector reviewed five (5) children records & four (4) staff records. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Licensing Inspector conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director/Administrator on March 5, 2019. Violations:
Standard #: 22VAC40-665-510-B
Description: Based on record review & discussion with the Director, the Vendor failed to maintain a written hard copy record of daily attendance for each group of children, that documents the arrival and departure of each child in care as it occurs. EVIDENCE: Documentation of daily attendance in each group was unavailable.

Plan of Correction: Director will create new attendance logs for each group of children

Standard #: 22VAC40-665-520-B-3
Description: Based on record review, the Vendor failed to have required information for each enrolled child. EVIDENCE: Licensing Inspector reviewed 5 children records. The work name, address and/or phone number for each child was not documented in the child's record.

Plan of Correction: Parents will add information.

Standard #: 22VAC40-665-530-2
Description: Based on record review, the Vendor failed to ensure staff member's submit to fingerprint-based criminal history check determination letter prior to the 1st day of employment. EVIDENCE: Licensing Inspector reviewed five (5) staff records. 2 out of the 5 staff members did not complete/submit fingerprint-based criminal history prior to the 1st day of employment. Staff #2 date of hire is 1/18/2001; fingerprint criminal history determination letter was completed on 2/06/2018. Staff #3 date of hire is 2/15/2018; fingerprint criminal history determination letter was completed on 11/20/2017.

Plan of Correction: Director will submit proof of completion within 10 days. Staff will not work until completed

Description: Based on record review, the Vendor failed to ensure each staff member completes a sworn statement or affirmation. EVIDENCE: Licensing Inspector reviewed 4 staff records. Record for Staff #3 did not include documentation of a sworn statement or affirmation.

Description: Based on record review, the Vendor failed to ensure each staff member is certified in first aid and cardiopulmonary resuscitation. EVIDENCE: Licensing Inspector reviewed 4 staff records. Staff #4has not have first aid & cpr certification.

Plan of Correction: Staff will complete training certification.

Standard #: 22VAC40-665-650-E
Description: Based on record review, the Vendor failed to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. EVIDENCE: Center has not developed nor implemented a continuity plan.

Description: Based on inspection, the Vendor failed to ensure during designated rest period the ratio of staff to children over 24 months of age only may be double the number of children to each staff required by subsection C of this section. EVIDENCE: During designed rest/nap time children under the age of 24 months were included in the ratio of staff-to-children. Care for children ages 1- 24 months were not not in a group exclusively for under 24 months of age.

Description: Based on inspection, the Vendor failed to practice at least 2 shelter-in-place drills annually. EVIDENCE: Center has not complete 2 shelter-in-place emergency drills over the past year.

Plan of Correction: Will complete 2 shelter-in-place drills annually.

Standard #: 22VAC40-665-780-A-3
Description: Based on inspection, the Vendor failed to practice 1 lock-down drill annually. EVIDENCE: Center has not complete 1 lock-down emergency drills over the past year.

Plan of Correction: Will complete emergency drill annually.
