Comments:
Today's inspection was a routine monitoring inspection. The inspection began at 11:00am and concluded at 2:30pm. There were six children in care with one provider and a total of twelve points. during the inspection three out of ten children's records were reviewed, the provider's record was reviewed, lunch time was observed, nap time was observed, the outdoor play area was viewed, some policies and procedures were reviewed, and hand washing was observed. Please ensure all menu substitutions are documented on today's menu. Risk assessment was discussed with the provider at the end of the inspection and is located on the Supplemental page of today's inspection. thnak you for your time, please call my office if you have any questions.

Violations:
Standard #: 22VAC40-111-900-A
Description: Based on the inspector's observations, the provider failed to prepare food in a manner that prevents choking. Evidence: The inspector observed lunch and hotdogs were served. The hot dogs were cut once length wise and then in half. The children in care were between the ages of two and three years. Hotdogs are required to be cut in a manner of no larger than 1/2 inches in diameter when served to children under the age of four years.

Plan of Correction: The provider will dice the hotdogs in the future when serving children under the age of four.

Standard #: 22VAC40-111-60-B
Description: Based on the inspector's review of three out of ten children's records, the provider failed to ensure a portion of the records contained all of the required information. Evidence : Child #1 with a start date of 9/2/14 does not have the name of one out of two required emergency contacts. The area for the parent to indicate chronic physical/developmental conditions was left blank.

Plan of Correction: The provider will obtain the information when the child is picked up at the end of the day. The provider will review the children's records to ensure they are complete.

Standard #: 22VAC40-111-70-A
Description: Based on the inspector's review of a sample of three out of ten children's record, and the provider failed to ensure required written information to parent's was provide before the child's first day of attendance. Evidence #1: The record for child #1 with a start date of 9/2/14 did not have documentation regarding whether or not the family day home has lability insurance acknowledged by the parent. Evidence #2: The record for child #2 with start date of 8/24/14 did did not have documentation regarding whether or not the family day home has lability insurance acknowledged by the parent. The same record did not include written acknowledgement by the parent regarding the FDH emergency preparedness plan. Evidence #3: The record for child #3 with a start date of 11/3/14 did not include written acknowledgement by the parent regarding the FDH emergency preparedness plan.

Plan of Correction: The provider will give the parent the required information and have the parent sign the model forms for the liability insurance disclosure and the provisions for the FDH emergency plan at the end of the program day upon pick-up of the child. The provider will review the additional children's records to ensure completeness.

Standard #: 22VAC40-111-90-A
Description: Based on the inspector's review of a sample of three out of ten children's records, the provider failed to ensure a child had current immunizations before their first day of attendance. Evidence: The record for child #2 had a documented start date of 8/2/14 and the immunization record was dated 8/26/14. Immunization records indicating the child is adequately immunized are required before a child may enter care.

Plan of Correction: The provider will not allow a child to start care without proper immunization records.
