An announced monitoring inspection was conducted on 11/24/2014. The LI arrived at nap time to find one caregiver with 28 points while the provider went to the store. The provider arrived home as the children were waking up from nap. Transition from nap to snack, potty time, diapering and hand washing were observed. Conversations about the upcoming holiday and family time were followed by singing songs about turkeys. The toddlers were engaged in gross motor activities and all children had access to an ample amount of toys located in different centers around the room. Standard #: 22VAC40-111-730-D
Description: Based on observation, medications for caregivers and household members, were not kept in a locked place using a safe locking method that prevents access by children. Evidence, the LI noted that antacids, vitamins, and supplements were sitting out on a low counter in the child care kitchen area. They were placed towards the back but were still within reach of taller children.

Plan of Correction: They will be kept in a locked place.

Description: Based on review of children's records, annual file reviews were not documented. Evidence, of 5 children's files reviewed, all were missing documentation that parents had reviewed required information.

Plan of Correction: The provider will have parents review files at the start of each summer when they go on field trips more often. The provider gas been doing this verbally and by email but has not documented in children's files.

Description: Based on observation during inspection, a bottle was propped for a sleeping infant. Evidence, an infant was asleep in a swing with a bottle propped up by two towel rolls.

Plan of Correction: The caregiver said this was only done to answer the door for the LI. Otherwise, this is not done and will not be done again.

Description: Based on observation upon LI's arrival, the caregiver exceeded 16 points at nap time. Evidence, The LI arrived at the Family Day Home at 2:40pm on 11/24/2014. The assistant notified the LI that the provider had just left to pick something up at the store. The children were asleep downstairs. There were a total of 28 points. The assistant called the provider and she arrived back at the Family Day Home at 3:00pm as the children were waking up.

Plan of Correction: The provider usually has a system in place in case she has to leave, which she states is not often. The provider will ensure that another caregiver is on site to ensure that the proper ratios are maintained.

Standard #: 22VAC40-111-690-A
Description: Based on observation, a child was left on the diaper changing table unattended. Evidence, a caregiver was changing a child's diaper on the changing table. The care giver finished and turned to attend to another child a few steps away. leaving the child on the changing table unattended.

Plan of Correction: The caregiver was made aware of the incident immediately and will make sure to keep a hand on the child at all time while on table.
