Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, November 25, 2014 from 10:00am to 11:45pm. The children were playing outside upon the inspector?s arrival, and they were later observed watching a reading dvd and eating snack. Snack consisted of animal crackers and grape juice. The caregivers were observed playing with the children outside and reading stories to them. After storytime the children had naptime. After naptime the children have lunch. On the menu today for lunch is spaghetti and meatballs, pineapples and milk. Today, there were four children, totaling eight points, present with two caregivers. Two children?s records and two caregivers? records were reviewed during the inspection. Last emergency drill: 10/24/2014 Last shelter-in-place drill: 5/28/2014 Emergency Plan Update: 12/23/2014 The provider has been approved for a special use permit, effective April of 2014. The violations from the previous inspection in June of 2014 were checked for corrections. There were no repeat violations found during today?s inspection. Description: Based on a review of children's records on 11/25/2014, the provider did not ensure that one of two children's records reviewed contained documentation of an annual update of emergency information. EVIDENCE: The most recent update documented in the record of Child #2 is dated 10/21/2013. Each child's record shall contain documentation of the annual review of the child's emergency contact information.

Plan of Correction: Will have parent update form at pick up today, 11/25/2014

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records on 11/25/14, the provider did not ensure that two of two children's records reviewed contained documentation of immunizations prior to the child's first day of attendance. EVIDENCE: 1. The earliest documentation of immunizations in the record of Child #1, whose first day of attendance at the family day home was 10/29/2014, is dated 11/18/2014. 2. The earliest documentation of immunizations in the record of Child #2, whose first day of attendance at the family day home was 09/16/2013, is dated 10/08/2013. Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Plan of Correction: From now on, I will require all immunizations before a child starts.
