Comments:
An unannounced monitoring/renewal inspection was conducted today from 10:40am-11:45am. There were 4 children (12 points) directly supervised by 1 caregiver. The home, 1 caregiver's records, 4 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, preparing for lunch and eating lunch. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Description: Based on review, the provider failed to complete an evacuation drill on a monthly basis. Evidence: Reviewed the provider's evacuation drill log and found that there was no drill completed during October.

Description: Based on review, the provider failed to ensure that all children have documentation of a physical examination within 30 days of their first day of attendance. Evidence: Reviewed 4 children's records and found that CH2 did not have documentation of a physical examination. CH2 has been in care since 9/2/14.

Description: Based on observation, the provider failed to wash her hands with liquid soap and warm running water after changing a child's diaper. Evidence: Observed that the provider did not wash her hands after changing the diaper of a child in care. The provider used a wet wipe and then continued to change the diapers of other children in care.
