Comments:
An unannounced renewal inspection was conducted from 10:27 a.m. - 1:15 p.m. There were five children in care with the provider and her assistant. The record for the assistant was reviewed and two children's records were reviewed. The children were observed playing with toys, working with gadget games, playing with baby dolls, looking at books, during redirection and time out, during story time, discussing the story and the main character, talking about the definition of words, identifying colors, counting while marching, identifying animals, playing pretend, identifying their body parts, singing while cleaning up, washing hands, saying grace, having lunch and during nap. Standard #: 22VAC40-111-240-A
Description: Based on inspection the facility failed to have removed from the yard trellis boards with exposed nails.

Evidence:

1. On 01/06/2015 the licensing inspector observed as entering the facility some trellis boards that had exposed nails in a black box near the sidewalk.

2. On 01/06/2015 the provider went outside with the licensing inspector and confirmed there were trellis boards with exposed nails in the black box near the sidewalk.

Plan of Correction: I was unaware that the person I had helping clean the yard had put the trellis boards in the black box. I immediately took these boards and put them in the garbage. I will be sure to monitor the outdoor area each morning.
