Technical Assistance:
Discussed with Provider the importance of having Background checks on-site and documented.

Today an unannounced monitoring visit was conducted with the Provider-Mrs. Grace Paniagua. This visit took place in the morning, there were 8 children with the Provider and 1 assistant for a total of 30 points within the supervision guidelines. The children were observed having morning breakfasts, nap time, free play, singing songs, playing with blocks. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-120-B
Description: Based on observation of the assistants records, the following information were not in the records: phone number of an emergency contact person, documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Caregiver #2's did not have the phone number for the emergency contact and only had one reference in her record.

Plan of Correction: Provider had the caregiver provide the phone to her emergency contact during this inspection. Provider will request another reference.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on observation of the provider records, one caregiver present did not have documentation of a Criminal record check and central registry check within 30 days of employment ends. Evidence: Caregiver #2, who started working at the family day home in August 2014 did not have documentation of a criminal record and central registry check.

Plan of Correction: Provider stated that she submitted the paperwork to the county and they misplaced it. Provider stated that she is currently in the process of resubmitting.
