54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted beginning at 12:30 p.m. and ending at 2:00 p.m. with the provider. There were three children in care ranging in age from three months to two years old for a total of ten points. One infant was observed sleeping. Another infant was observed waking, being diapered and fed. A two year old was observed having lunch which consisted of chicken nuggets, french fries, fruit cocktail, wheat bread and milk. After lunch, a child was observed being administered a nebulizer treatment before nap. Two children's records and one staff record was reviewed. The weekly menu was reviewed. Description: Based on observation, the weekly menu was not complete for 3/2/2015-3/6/2015.

Plan of Correction: I was following the advice of the food program. The menu will be completed as soon as possible.

Standard #: 22VAC40-111-960-E
Description: Based on observation, infant bottles were not labeled and dated as required. Evidence: With two infants in care, bottles were not dated and labeled.

Plan of Correction: Bottles will be labeled and dated. Labels were created during the inspection.

Standard #: 22VAC40-111-120-B
Description: Based on review of records, not all provider records were complete. Evidence: Staff A, start date 11/12/2014, did not have documentation of two references.

Plan of Correction: References will be documented and put in the file.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering pad was absorbent. Evidence: The diapering pad had three tears in the nonabsorbent cover exposing the absorbent material underneath.

Plan of Correction: I will purchase a new diapering pad.
