Today an unannounced renewal inspection was conducted with the Provider-Ms. Munoz. This visit took place in the morning between 8:00-11, there were 1 child present for a total of points within the supervision guidelines. The child was observed having morning breakfast, singing songs, listening to music, learning body parts and during free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance, menu and emergency procedures. Standard #: 22VAC40-111-800-B Description: Based on observation of the emergency plan, the emergency plan did not include documentation that it had been updated annually. Evidence: The emergency plan yearly review for fiscal year 2015 had not been documented or completed. Plan of Correction: Provider stated that she will review the emergency plan and update the form.
Standard #: 22VAC40-111-100-A Description: Based upon documentation reviewed, a child's physical examination was not obtained. Evidence: One of the four children's records did not contain a physical examination prior to the child's attendance or within 30 days after the first day of attendance (Child B; DOE 10/01/2011). Plan of Correction: Provider stated that the child had left last year and came back two months ago. Provider will inform the parent to provide the information.
Standard #: 22VAC40-111-110-B Description: Based upon documentation reviewed, a child's immunizations record did not meet requirements. Evidence: One of the four children's records contained immunizations record that were not signed by a licensed physician, the physician's designee, or an official of a local health department (Child B). Plan of Correction: Provider stated that the child had left last year and came back two months ago. Provider will inform the parent to provide the information.
Standard #: 22VAC40-111-60-A Description: Based on a review of the children's records, one record had not been updated by the parents yearly. Evidence: Child c's record, did not include documentation that the parent reviewed and updated the record since 12/13. Plan of Correction: Provider will notify the parent and have them review the record and sign the form.
