Comments:
The inspector arrived at 8:45 AM and departed at 10:30 AM. Six children, totalling 13 points were in care with one caregiver. Morning activities, breakfast service and preparation for special Easter activities were observed. A sampling of three children's records were reviewed. Caregiver and household records were checked for updates. Medication administration records and procedures were reviewed. No children's injuries were reported. The provider's plans of correction were obtained and completed during the exit meeting prior to concluding the inspection. Please contact the inspector at (757) 491-3975 if there are any questions.

Violations:
Standard #: 22VAC40-111-700-C
Description: Based upon review of medication, the provider has not ensured that when an authorization for medication expires, the parent is notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Evidence: An albuterol inhaler is stored at the family day home for possible administration in an emergency. The physician authorization for the medication to be administered expired on 02/14/2015.

Plan of Correction: The parent was contacted during the inspection to obtain an updated authorization from the physician.

Standard #: 22VAC40-111-60-B
Description: Based on review of three children's records, the provider has not ensured that children's records include the names, addresses and telephone numbers of two designated persons to contact in case of an emergency if the parent cannot be reached. Evidence: The record for child 3 does not include the address for one of the two emergency contact persons.

Plan of Correction: The parent will be contacted today to add the complete street address for the emergency contact person.
