An unannounced monitoring inspection was conducted today. The Provider had five children in care, 14 points. The children were observed during free play time. The Provider has seventeen and a half hours of training at the time of inspection. There are no pets in the home. The children were having macaroni and cheese, grapes, apples and milk for lunch. The inspection began at 10am and ended at 11:45. Three files reviewed and two interviews conducted.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, it was determined that the Provider did not have a completed record for a child in care. Evidence: Child A's record was missing a second emergency contact, persons authorized to pick up the child, and allergies.

Plan of Correction: The record will be completed today by the parent.

Standard #: 22VAC40-111-340-C
Description: Based on direct observation by the Licensing Inspector, the bathroom was missing items for the children. Evidence: The Provider failed to have paper towels in the bathroom for the children in care to dry their hands.

Plan of Correction: The Provider placed the paper towels in the bathroom as required.
