The renewal application can be obtained from the DSS web site. The application consists of three parts: 1. Renewal application for licensure of a child welfare agency. 2. Part II: Program addendum to application for licensure of a family day home. 3. Staff information sheet. Please make sure all three parts of the application are submitted with the appropriate licensing fee.
An unannounced renewal inspection was conducted on 3/31/2015 from 9:25am - 11:25am. At the time of entrance, there were five children in care with two caregivers. The ages of the children counted for a total of 14 points. Children were observed engaged in play activities with the assistant. One diaper change was observed. The provider was observed holding and attending to the one infant in care. Morning snack was served which consisted of animal crackers and milk. Appropriate hand washing was observed through out the inspection. Records were reviewed for five children and two staff. Emergency supplies and practice logs reviewed. *No medication is being administered at this time. The entire home and backyard play area was reviewed. Please complete the "plan of correction" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-111-210-A Description: Based on record review, the provider failed to ensure that the designated assistant shall obtain a minimum of 16 clock hours of training in areas relevant to their job responsibilities. Evidence: The assistant has documentation of 8 hours of annual training for 2014. Four hours of training has been obtained for 2015. Plan of Correction: My assistant will start her trainings. The first 3 hour training is on April 9, 2015.
Standard #: 22VAC40-111-280 Description: Based on observation, the provider failed to ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents and disinfectants, shall be stored away from food in areas inaccessible to children. Evidence: 1. A closed container of germicidal disposable wipes were observed stored on top of the toilet in the bathroom used by children. The product label contained multiple warning labels indicating the product was a hazardous chemical. 2. This area is accessible to children and children were observed using the bathroom during the inspection. Children were supervised however. Plan of Correction: I removed the disinfectant wipes and put them under the cabinet out of the reach of the children.
