Discussion was held on the topics of current enrollment and current staffing.

Comments:
The following inspection took place between the hours of 11 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children (18 points) with 2 caregivers. A total of 5 new children?s files and 6 staff/household member?s files were reviewed. The children were engaged in the following: free play, toy play, games, music, lunch, bathroom breaks, hand washing, and other organized activities. Violations:
Standard #: 22VAC40-111-200-A
Description: Based on review and interview, 1 of 6 staff/household member's files reviewed indicated that documentation of an orientation was not available. Evidence - 1. On the date of inspection (04/15/2015), the document of an orientation for Staff #2 was not available for review. 2. The Provider stated (during an interview conducted on the date of inspection) that an orientation was conducted but the documentation could not be located.

Plan of Correction: Corrected on the date of inspection by completing new form.
