An unannounced monitoring inspection was conducted on April 14, 2015 from 10:20 a.m. to 12:15 p.m. During the inspection, there were 5 children ages 8 months to 5 years with 1 caregiver present. Children were observed doing puzzles, listening to a story while demonstrating actions and sounds from the story, doing exercises, assembling items using interlocking pieces, writing numbers and letters on a chalk board, making and cutting paper designs, conversing with each other and their caregiver. Records were reviewed for 3 children and 1 caregiver. There was 1 medication review. Areas of non-compliance are identified on the violation notice and were discussed with the program director during the exit interview. Please complete the ?plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring and preventive measure(s). Standard #: 22VAC40-111-790
Description: Based on observation and interview, the facility did not ensure that the following telephone numbers were posted in a visible area close to the telephone: 1. A 911 or local dial number for police, fire, and emergency medical responders; 2. The responsible person for emergency backup care as required in 22 VAC 40 111 800 A 3; and 3. The regional poison control center. Evidence: The emergency telephone numbers were not posted which the provider confirmed .

Plan of Correction: Provider will post in a visible area close to the telephone all emergency numbers .Police, non-emergency and emergency responders, and the number and name of a emergenecy [sic] backup care person, and the number of the Regional Poison Control center number.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the facility did not ensure that areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Evidence: 1. There was an unsafe condition in the children's bathroom because a container of baby powder was set on a shelf beside the toilet within reach of a 5 year old child, in care, who can be permitted privacy when toileting. 2. The baby powder was labeled: "Keep out of reach of children. Keep away from a child's face to avoid inhalation which can cause breathing problems. Avoid contact with eyes."

Plan of Correction: the provider will remove the labeled container of baby powder out of bathroom .
