Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:00am - 11:30am. There were two children in care for a total of seven points with one caregiver. The children were observed at free play, during snacks and at story time. One adult and two children's records were reviewed. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-100-A
Description: Based on record reviews, the family day home failed to obtain documentation of a physical examination. Evidence: 1. The inspector reviewed two children's records. Two of two records reviewed (children's records #1 and #2) did not contain documentation of a physical examination. Child #1's date of enrollment was 3-9-15 and child #2's date of enrollment was 2-27-15.

Plan of Correction: The provider will remind the children's parents that documentation of a physical exam is needed for the children's records. The provider will review the children's records quarterly to ensure all contain documentation of a physical exam.

Standard #: 22VAC40-111-70-A
Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed two children's records. Two of two records reviewed (children's records #1 and #2) did not contain the policy number for the medical insurance.

Plan of Correction: The provider will request this information from the children's parents. The provider will review the children's records quarterly to ensure all information is complete.

Standard #: 22VAC40-111-90-A
Description: Based on record reviews, the family day home failed to obtain documentation of immunizations. Evidence: 1. The inspector reviewed two children's records. One of two records reviewed (child record #1) did not contain documentation of immunizations.

Plan of Correction: The provider has requested a copy of the child's immunizations from the child's parents and is expecting this documentation by Friday. The provider will review the children's records quarterly to ensure all information is complete.

Standard #: 22VAC40-111-350-E
Description: Based on observation, the family day home failed to maintain the hot water temperature within the acceptable range of 105 to 120 degrees. Evidence: 1. The inspector used the thermometer to measure the hot water temperature. The hot water temperature measured 128 degrees.

Plan of Correction: The provider will have the temperature of the hot water heater turned down. After the heater is turned down, the provider will measure the temperature of the hot water to ensure it does not exceed 120 degrees.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the family day home failed to have the children wash their hands. Evidence: 1. The inspector observed the children at snack time. The children did not wash their hands before eating snacks.

Plan of Correction: The provider will monitor the children more closely before eating to ensure they wash their hands with soap and warm running water.
