Technical Assistance:
Discussed the requirements for an acceptable physical exam report for enrolled children.

An unannounced full renewal inspection was conducted today. The provider was caring for three children, totaling 9 points. Two of the children present today were the provider's own children. The children were observed playing with toys, engaging in creative play, coloring pictures, and playing hide and seek. Pleasant and inviting interactions between the provider and all children in care were observed. The children were to be served peanut butter and jelly sandwiches, sliced bananas, and milk for lunch later today. This inspection was started at approximately 9:15 am. Thank you for meeting with me today. If you have any questions regarding this inspection please email me at

Standard #: 22VAC40-111-830-B
Description: Based on a review of emergency drill documentation, it was determined that the provider did not conduct at least 2 shelter-in-place drills during the 2014 calendar year. Evidence: The provider conducted only 1 shelter-in-place drill in 2014.

Plan of Correction: I will do 3 shelter drills this year.

Standard #: 22VAC40-111-830-C
Description: Based on a review of emergency drill documentation, it was determined that the provider did not record the fire evacuation drill each month. Evidence: There is no documentation to show that the provider conducted a fire evacuation drill during the month of March, 2015.

Plan of Correction: We did the fire drill but I forgot to write it down. I will remember to record the fire drill each time.

Standard #: 22VAC40-111-290
Description: Based on observations, it was determined that sharp kitchen utensils were not kept inaccessible to the children. Evidence: In the kitchen, two pizza cutters and a can opener were stored inside a drawer. The child safety lock on this drawer is broken and no longer locks.

Plan of Correction: I will replace the safety lock.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
