Technical Assistance:

An unannounced monitoring inspection was made on 4/21/2015 at 9:00 a. m. and concluded at 10:45 a. m. The inspector met with the provider and Family Day Home standards were reviewed. The following staff-to-child ratios and activities were observed today. There were 12 family day home children plus the provider's child present with points equaling 32 with two caregivers providing supervision. The provider was conducting circle time with the children, the inspector observed the children singing songs, going over the month, year, and days of the week, they talked about Earth Day, and what they could do to help our Earth. The children were working on letter "U", and did an activity sheet where they circled the letter "U" and wrote the letter "U". Two children records were reviewed. Caregiver and household member records were reviewed. One observation conducted. Conversations were held with two children and two caregivers. No medications are given. The last emergency monthly evacuation drill was conducted on 4/13/15. The last shelter-in-place drill was conducted on 3/17/15.

Description: Based on review of children records the provider did not have all required information on file. Evidence: On 4/21/15, there was no documentation available for proof of identity and age for child #1 (whose start date is 2/9/15).

Plan of Correction: Mother of child #1 will bring it back to me on Wednesday, April 22, 2015.

Standard #: 22VAC40-111-180-A
Description: Based on review of TB screenings the provider did not have one TB screening updated within two years. Evidence: On 4/21/15, household member #1 has a TB screening dated 4/1/2013. TB screenings are required to updated every two years.

Plan of Correction: Household member #1 will go today to get TB test.
