The application for renewal of the provider's license was discussed. The provider had planned to mail the application to the Division of Licensing Programs office this week. The required pages of the application and addendum were reviewed with the provider to ensure all parts of the application were completed when sent.
An unannounced renewal inspection was completed on 4/20/15. There were four children present at arrival with the assistant/substitute provider supervising. The point total was 10. The provider arrived at a few minutes after 10:00 am. Two additional children arrived after the provider. The point total with all six children from ages 18 months through three years was 14. The provider and assistant were supervising during the rest of the inspection. Five newly enrolled children's records were reviewed and five children's records were reviewed for annual updates only. The provider's record, the assistant's record and one household member's record were reviewed. The inspector arrived at the provider's home at 9:45 am and departed at 2:00 pm. The children were observed during free play time, during outside time, during lunch, and during nap. There was discussion with the provider about upcoming updates. There was also discussion about the types of licenses that licensees can receive (one year, two year, three year) and the way in which this is determined.
Standard #: 22VAC40-111-70-A Description: Based on record review and interview with the provider, the provider failed to ensure that the parents of five of five newly enrolled children were provided with all required information in writing prior to the childrens' first day of attending. Evidence: 1. The records of children #6,7,8,9,10 were missing 10 of the 19 required policies/procedures to be given to parents in writing prior to the child attending. The policies that were not provided to the parents were: requirements for notification of a sick child and parent to pick up, requirement for parents to inform the provider within 24 hours of child or anyone in household having a reportable communicable disease, requirement for the child to be adequately immunized, requirement for caregivers to report child abuse or neglect, custodial parent's rights to be admitted to the family day home, policies for the provision of food, presence of a pet or animal in the home, discipline policies, address of the web site of the Department with a note that compliance history and regulations are available on the web site. The first page of the "Information for Parents" form was not in the childrens' records and was not a part of the packet of forms for parents to complete at enrollment. Plan of Correction: The provider will print the first page of the "Information for Parents" form and have all parents sign that they have been provided with the information. For future, the provider will add page one to the enrollment packet for use with all newly enrolled children.
Standard #: 22VAC40-111-680-B Description: Based on observation, the provider failed to ensure that children's hands were washed as required. Evidence: 1. Two diaper changes were observed after lunch. The two children did not have their hands washed after being diapered as required. Plan of Correction: The provider will make sure the children wash their hands after each diaper change.
