An unannounced monitoring inspection took place this afternoon from 1:25 - 2:45 pm. There were five children present with the provider and assistant for 10 points. Schoolagers will arrive after school. Children were all asleep during naptime. The assistant provider reported that the playground in the backyard is not yet in use and he is not currently providing transportation. There was no medication present for any of the children today. Records were well-organized. If you have any questions, please contact me at firstname.lastname@example.org **Please note: my phone number has been changed to 703 304-9609. The main office number remains 703 934-1550.
Standard #: 22VAC40-111-60-B Description: A review of five children's files today found some required information for one child was not documented. Evidence: Of five files reviewed today, Child # 5 (first date of attendance was 9/14) did not have complete emergency contact information or complete health insurance information. Plan of Correction: The parent said she did not have anyone for an emergency contact, but we will ask again. We will also find out about health insurance for the child.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
