Comments:
An unannounced monitoring inspection took place this afternoon from 1:25 - 2:45 pm. There were five children present with the provider and assistant for 10 points. Schoolagers will arrive after school. Children were all asleep during naptime. The assistant provider reported that the playground in the backyard is not yet in use and he is not currently providing transportation. There was no medication present for any of the children today. Standard #: 22VAC40-111-60-B
Description: A review of five children's files today found some required information for one child was not documented. Evidence: Of five files reviewed today, Child # 5 (first date of attendance was 9/14) did not have complete emergency contact information or complete health insurance information.

Plan of Correction: The parent said she did not have anyone for an emergency contact, but we will ask again. We will also find out about health insurance for the child.
