- Technical Assistance:

An unannounced monitoring inspection was conducted today from 11 am - 1:15 pm. There were 12 children in care with the provider and two assistants for 31 points. I observed children playing outdoors on play equipment; eating a picnic lunch; brushing teeth; listening to a bedtime story and going to naptime. Record keeping was well-organized. A sign on the outside door welcomed a new child and said good-bye to a child who would be leaving after several years. Additionally, a variety of planned theme activities according to the month were observed, boxed and ready to bring out for learning activities. There were no children's medications on site today. If you have any questions, please contact me at firstname.lastname@example.org **Please note: My phone number has been changed to 703 304-9609. The main office number remains 703 934-1505.
Standard #: 22VAC40-111-780-B Description: A review of children's records today found that some did not have documentation of annual updates of parent emergency contacts. Evidence: There has been no update for Child # 1 since 2/14; and none for Child # 2 since 1/14. Plan of Correction: I will have the parents review it today.
Standard #: 22VAC40-111-240-A Description: Some areas of the family day home were found today to not be maintained in a safe or operable condition. Evidence: Outdoors: There was an area where the lattice work from the fence was coming off at the bottom of the fence; a few areas of splintering and one back of a small nail was protruding. Downstairs: There were two uncovered light bulbs in the ceiling. Plan of Correction: The small nail was immediately covered with paper and duct tape. The lattice will be nailed closed and areas will be sanded. The lights in the baby room will be covered or installed so they will not shatter if broken.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
