54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 9:30am to 10:35am. At the time of inspector's arrival there were 4 children, ages 1-2yrs, for a total of 11 points, in care with the provider. The records sample size consisted of 4 children's records and 3 provider / assistant/ household member records. Children were observed engaging in creative play and departure. The provider stated that no medication is administered to children while in her care. The provider also reported there were no incident reports since the last inspection.
Standard #: 22VAC40-111-780-B Description: Based on record review and interview, it was determined that the provider did not ensure that annually the provider review the emergency contact information and have the parent sign to acknowledge the review. Evidence: The record for Child#4(enrolled 1/6/14) did not contain documentation of an annual review of the child's emergency contact information. The provider could not provide this documentation during the inspection. Plan of Correction: Provider will review information in child's record with parent and have parent sign and date the annual review at pick up today.
